The tear trough is the groove between where the lower eyelid and cheeks meet. One of the first noticeable signs of aging is the appearance and gradual worsening of under eye dark circles, due to thinning of the overlying skin and soft tissue over the bony orbital rim. A band of tethering fibrous tissue called the orbitomalar ligament binds the skin beneath the lower eyelid to the underlying rim of the eye socket. Because of surrounding fat and thicker skin surrounding this area, this ligament tends to be hidden when we are younger. As we get older, due to age atrophy of the adjacent tissues and thinning of the skin, this deep attachment between the skin and orbital rim becomes more and more visible.
